If your child has diabetes, seizures, asthma or a bee or food allergy, please print and complete the appropriate form(s) below and return them to your child's school office. These action plans will be kept on file for one school year for use in the event your child has a medical emergency.

Also, please remember you must have a medication administration form signed by a physician on file at school for any medicine that is to be given at school.

New forms must be completed each school year for both the action plans and medication administration forms.

Thank you for your cooperation to assure your child has a safe and healthy school year. If you have any questions, please call your school's office staff and they will forward a message to the school nurse.
